Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate Registered Nurse for a PRN field position serving patients throughout Fort Worth and surrounding communities. You will provide skilled nursing visits in patients' homes, develop care plans, and coordinate with physicians and caregivers to ensure safe, high‑quality care.

The role requires an active Texas RN license, CPR/BLS certification, and the ability to travel locally.

Qualifications

• Active Texas RN license.
• Current CPR/BLS certification.
• Minimum 1 year home health, hospice, or palliative care experience preferred.
• Ability to travel locally and work independently.

Responsibilities

• Perform skilled nursing visits at patients' homes.
• Complete Start of Care, Recertification, and discharge visits as assigned.
• Conduct comprehensive patient assessments and develop care plans.
• Administer medications per physician orders and coordinate with the care team.
• Document accurately in OASIS and EMR.
• Travel across Fort Worth area and surrounding communities.

Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dependable Registered Nurse for a PRN field position serving patients throughout Fort Worth and surrounding communities.

Our Registered Nurses provide skilled, patient-centered care in the comfort of each patient’s home. This position helps patients safely manage their health conditions while supporting their independence and quality of life.

Responsibilities

• Perform skilled nursing visits in patients’ homes
• Complete Start of Care, Resumption of Care, recertification, routine, evaluation, and discharge visits as assigned
• Conduct comprehensive patient assessments
• Develop and update individualized plans of care
• Administer medications and treatments according to physician orders
• Provide wound care, IV therapy, disease management, and other skilled nursing services as ordered
• Educate patients and caregivers regarding diagnoses, medications, treatments, safety, and care plans
• Monitor patients for changes in condition and promptly communicate concernsCoordinate care with physicians, caregivers, therapists, and the interdisciplinary team
• Complete accurate and timely OASIS and clinical documentation
• Follow agency policies and applicable Medicare, state, and federal requirements
• Travel to patients throughout DeSoto, Duncanville, Oak Cliff, Lancaster, and surrounding areas

Qualifications

• Active and unrestricted Texas Registered Nurse license
• Current CPR or BLS certification
• At least one year of nursing experience preferred
• Home health, hospice, or palliative care experience preferred
• OASIS and WellSky/Kinnser experience is a plus
• Strong assessment, clinical decision-making, and time-management skills
• Proficiency with electronic medical record systems
• Valid Texas driver’s license
• Reliable transportation and current automobile insurance
• Ability to work independently and collaborate with the clinical team
