Job summary

Boost Home Healthcare is seeking a Field Registered Nurse (Field RN) to administer skilled nursing care to patients in their homes in Avon, OH. You will follow physician orders and the plan of care under the direction of the Director of Patient Care Services.

The ideal candidate is a licensed RN with at least 1 year of home health experience, current BLS certification, reliable transportation, and a passion for helping patients live independently at home.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license valid in the state of the posting.
• Minimum of one year of nursing experience in a home health setting.
• Must have current BLS certification.
• Access to reliable transportation.
• Preferably resides in Strongsville, Brook Park, Parma, Seven Hills or Independence area.

Responsibilities

• Make the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide those services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.
