Responsibilities

• Provide skilled physical therapy services in patients’ homes under the supervision of a Physical Therapist.
• Implement individualized treatment plans to improve mobility, strength, balance, and function.
• Assist patients with therapeutic exercises and functional mobility training.
• Educate patients and caregivers on safe movement and home exercise programs.
• Monitor and report patient progress.
• Document visits timely and accurately.
• Collaborate with the interdisciplinary team to ensure coordinated care.
