Activa Home Health in Boynton Beach, FL is seeking a Field Physical Therapist Assistant to provide in-home PT services under a supervising PT. The role offers flexible per diem scheduling with potential for permanent placement.

You’ll deliver targeted exercises, educate patients and caregivers, monitor progress, and collaborate with the care team to ensure safe, coordinated care. Active PTA license in Florida and CPR certification are required.

Qualifications

• Active Florida PTA License.
• Degree from an accredited PTA program.
• Current CPR Certification.
• Valid driver’s license and reliable transportation.
• Strong clinical and communication skills.
• Home health experience preferred.
• Medicare experience preferred.

Responsibilities

• Provide skilled physical therapy services in patients' homes under a Physical Therapist.
• Implement individualized treatment plans to improve mobility, strength, balance, and function.
• Assist patients with therapeutic exercises and functional mobility training.
• Educate patients and caregivers on safe movement and home exercise programs.
• Monitor and report patient progress.
• Document visits timely and accurately.
• Collaborate with the interdisciplinary team to ensure coordinated care.
