Activa Home Health is seeking a Field Physical Therapist Assistant to support patients in Boynton Beach, FL on a flexible per diem basis with potential for permanent placement. You will work under a Physical Therapist to implement treatment plans and improve mobility and function in patients’ homes.

The role emphasizes ongoing education, weekly pay, and strong clinical leadership, with opportunities for growth and recognition within a supportive team.

Qualifications

• Active Florida PTA license.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled physical therapy in patients’ homes under supervision of a PT.
• Implement individualized treatment plans to improve mobility, strength, balance, and function.
• Assist patients with therapeutic exercises and functional mobility training.
• Educate patients and caregivers on safe movement and home exercise programs.
• Monitor, document, and report patient progress timely.
• Collaborate with interdisciplinary team for coordinated care.

What You’ll Do

• Provide skilled physical therapy services in patients’ homes under the supervision of a Physical Therapist

What You’ll Need

• Home health experience preferred
• Medicare experience preferred
