Activa Home Health is seeking a Field Physical Therapist Assistant for a flexible per diem role in Boynton Beach, FL. The PTA will provide home-based therapy under a supervising PT and may pursue permanent placement.

Responsibilities include implementing treatment plans, guiding patients through exercises, educating families, and documenting progress. Florida PTA license, CPR, and reliable transportation are required; home health and Medicare experience are preferred.

Qualifications

• Active Florida PTA license
• Degree from an accredited PTA program
• Current CPR certification
• Valid driver’s license and reliable transportation
• Strong clinical and communication skills
• Home health experience preferred
• Medicare experience preferred

Responsibilities

• Provide skilled physical therapy services in patients’ homes under the supervision of a Physical Therapist
• Implement individualized treatment plans to improve mobility, strength, balance, and function
• Assist patients with therapeutic exercises and functional mobility training
• Educate patients and caregivers on safe movement and home exercise programs
• Monitor and report patient progress; document visits timely and accurately
• Collaborate with the interdisciplinary team to ensure coordinated care
