Job summary

21st Century Home Health Services is hiring a Health Field Physical Therapist for home visits in the Santa Cruz area. This entry-level role emphasizes building core clinical skills, with supervision by a PT Case Manager and strong leadership support.

Expect to participate in weekly interdisciplinary conferences, maintain patient privacy, and stay current with compliance training and best practices in home health.

Qualifications

• Current, unrestricted California Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new grads may be considered.

Responsibilities

• Provide hands-on therapeutic interventions including gait training, balance and neuromuscular re-education, transfer training, therapeutic exercises, manual therapy, and pain management.
• Assist patients with devices and home safety equipment.
• Conduct detailed patient evaluations in home environments.
• Implement plans of care under supervision of a PT Case Manager.
• Document patient care within 24–48 hours per care plan.
• Educate patients and families on mobility, exercises, safety, and equipment use.
• Collaborate with PT Case Manager to prepare for treatments and update care plans.
• Coordinate with RN, OT, ST, MSW, HHA for interdisciplinary care.
• Maintain cost awareness and appropriate use of equipment.
• Participate in ongoing professional development and training.
