Job summary

Adaptive Home Health is seeking an Occupational Therapist (OT) to work in the field across Texas. The role is full-time with Monday–Friday schedule and occasional weekends as needed, providing in-home evaluations and skilled visits.

You will develop plans of care focused on ADLs/IADLs, safety, and independence, and utilize Homecare Homebase for documentation. Requirements include an active Texas OT license, strong clinical judgment, and willingness to travel within the assigned territory.

Qualifications

• Active Texas OT license (in good standing)
• Strong clinical judgment and comfort working independently in the field
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly
• Strong communication and documentation
• Reliable follow-through on orders, frequencies, and patient goals
• Current driver’s license and comfort with daily travel across assigned territory
• Able to carry/transport basic therapy equipment as needed
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.
• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team when condition or needs change.
• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
