Qualifications
- Active, un-encumbered license to practice nursing in the state of assignment
- Nurse practitioner certification (ANCC or AANP)
- DEA license or ability to obtain
- Basic Cardiac Life Support (BCLS) certification
- Reliable transportation enabling travel to client/patient sites
Responsibilities
- Perform in-home assessments, obtain medical history, conduct physicals, and develop plan of care
- Identify gaps in care, interpret tests, and refer appropriately
- Assess risk factors and facilitate access to care
- Create interventions to meet care goals and monitor progress
- Communicate with patient, family and care team to coordinate care
- Travel 75% within local market to patient homes
- Collaborate with physicians and interdisciplinary team to deliver patient-centered care
- Explain options to patients and families to aid informed decisions
