Job summary

Advantmed, LLC is seeking a licensed Nurse Practitioner for a flexible role conducting in-home wellness and risk adjustment assessments for Medicare members in Ohio. This 1099 PRN position offers ~$100 per completed assessment plus mileage, ensuring reliable and ongoing supplemental income. You’ll perform patient evaluations, educate members, and document care in EMR systems. Ideal candidates will have an active NP license, three years of experience, and comfort with home assessments while maintaining professionalism and accountability.

Qualifications

• Minimum of 3 years of patient care experience preferred.
• Ability to work independently and manage time effectively.
• Comfort with electronic medical records systems.

Responsibilities

• Conduct in home Annual Wellness Visits and chronic condition assessments.
• Document visits in EMR supporting value-based care.
• Educate members on preventive care and chronic disease management.
