Job summary

Advantmed is looking for a licensed Nurse Practitioner in Ohio to conduct structured, in-home wellness assessments for Medicare members. This role offers flexible weekdays without evenings or weekends, with compensation of around $100 per completed assessment plus mileage. Candidates should have an active NP license and preferably 3 years of patient care experience. The role requires availability for approximately 30 hours per month and offers operational support for scheduling and tech needs.

Qualifications

• Active, unencumbered NP license in Ohio.
• 3+ years of patient care experience preferred.
• Comfortable with EMR systems and independent work.

Responsibilities

• Conduct in home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
• Educate members on preventive care and chronic disease management.
