Job summary

SCAN is seeking a Licensed Vocational Nurse (LVN) in California to provide direct, on-site and remote care to Embrace members in assisted living and community-based settings. You will work under NP/MD direction, performing focused assessments, wound care, vaccinations, IV therapy within scope, and catheter care.

The LVN will be a visible brand ambassador, collaborating with community staff and the primary care team to ensure high-quality care delivery and adherence to workflows.

Qualifications

• LVN license required.
• Associate's degree or equivalent experience preferred.
• 3–5 years of related nursing experience.
• Works under direct supervision; escalates more complex issues to higher-level staff.

Responsibilities

• Provide direct, onsite and remote care to Embrace members residing in assisted living and community-based settings under NP/MD direction.
• Perform focused assessments including vitals, pain, mental status, and basic cardiopulmonary/GI observations; monitor skin integrity and wound status; catheter function.
• Administer vaccinations and IM/SC injections as ordered; perform wound care and routine foot care; catheter care and IV maintenance within scope.
• Coordinate with primary care team and community staff; maintain strong relationships with residents and families; contribute to transitions of care and medication support.
