Job summary

VNA Health in California seeks an LVN to provide direct patient care under RN supervision in a home health setting. You will follow the plan of care, document accurately, and collaborate with the care team to achieve positive patient outcomes.

Qualifications include California LVN license, CPR, and a valid driver's license with insured vehicle. Prior home health or medical/surgical experience is preferred; weekend rotation may be required.

Qualifications

• Current and active California LVN license.
• Minimum one year experience; community/home health or medical/surgical preferred.
• 3-4 years of LVN experience preferred.
• Current CPR certification.
• Licensed driver with insured automobile per state requirements.
• Complies with professional standards and practice.
• Demonstrates good verbal and written communication and organization skills.

Responsibilities

• Provides direct patient care as defined in the State Nurse Practice Act.
• Implements plan of care initiated by the clinician.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and educates patients and families about diagnosis and plan of care.
• Coordinates home health services and communicates needs to the clinician and supervisor.
• Uses equipment and supplies effectively and efficiently.
• Participates in Quality Management program.
• Engages in professional growth and development.
• Meets daily productivity standards as set by the agency.
• Knows applicable regulations for home health services.
• Performs other duties as assigned by the RN.
