Job summary

SCAN Health Plan seeks an LVN to provide direct, onsite and remote care to Embrace members in assisted living and community-based settings under NP/MD direction. Focus on assessments, wound care, vaccination support, IV therapy, and catheter care within scope.

You will act as a brand ambassador, collaborate with community staff, and support high-quality care delivery across assigned communities, with emphasis on safety and patient satisfaction.

Qualifications

• Licensed Vocational Nurse (LVN) with current license.
• 3–5 years of related clinical experience in home/field care or clinic settings.
• Ability to perform focused assessments and routine wound/IV care within scope.

Responsibilities

• Perform focused assessments and vital signs during rounds.
• Administer vaccinations and injections per protocol.
• Provide wound care, catheter care, IV hydration and related tasks.
• Coordinate with NP/MD and community staff for transitions of care.
