Job summary

SCAN is a nonprofit health organization serving members across several states. The LVN provides direct onsite and remote care to Embrace members in assisted living and community settings under NP/MD direction.

This role focuses on focused assessments, wound care, vaccinations, IV therapy within scope, catheter care, and close collaboration with community staff, delivering high-quality care and engaging with residents and families.

Qualifications

• Licensed Vocational Nurse (LVN) required.
• Associate's Degree or equivalent experience preferred.
• 3-5 years of related experience.

Responsibilities

• Perform focused assessments including vitals and basic observations.
• Administer vaccinations and injections per protocol.
• Provide wound care and catheter care within LVN scope.
• Coordinate care with NP/MD and community staff.
