Qualifications

• Bachelor’s degree with 2–4 years of related experience, or equivalent combination.
• Licensed clinician: NC LCSW or RN required.
• Experience in LTSS/HCBS coordination or case management preferred.

Responsibilities

• Evaluate member needs and available resources; plan for best outcomes.
• Develop long-term care plans and identify providers and community resources.
• Coordinate between members, families, caregivers and care team for access to services.
• Provide resource support on employment, housing, and living arrangements.
• Monitor plans and member status; adjust care plans as needed.
• Collaborate with healthcare providers to meet member needs.
