Job summary

Molina Healthcare is seeking a TX LVN Care Manager to perform in-home, face-to-face assessments for Medicaid members in Katy and Stafford within the Houston service area. Hours are Mon-Fri 8am-5pm CST; mileage is reimbursed. Bilingual applicants are preferred.

Qualifications

• 2+ years healthcare experience with LTSS and care management.
• Valid and unrestricted driver license with reliable transportation.
• Bilingual required to serve Medicaid members.

Responsibilities

• Completes comprehensive member assessments within timelines, including in-person home visits.
• Develops and implements waiver service plans with members and care teams.
• Monitors care plans, documents interventions, and evaluates goal achievement.
• Promotes integration of LTSS, behavioral health, and community resources.
• Facilitates ICT meetings and authorizes waiver services as needed.
• Coordinates with licensed care managers and travels locally 25–40%.
