Job summary

BayCare HomeCare seeks a dedicated Home Health Field LPN to provide in-home nursing care in the Largo area. You will deliver hands-on patient care, follow plans of care, and support patients to maintain independence during home visits.

Requirements include an active Florida LPN license, BLS, a valid FL driver's license, and 1 year of home care experience. The role offers a full-time schedule with weekday hours in Pasco County surrounding areas.

Qualifications

• BLS certification required.
• LPN license active in Florida.
• Florida driver's license within 30 days of hire.
• Associate's or Diploma in Nursing is required.
• 1 year of Licensed Practical Nursing experience.
• 1 year of Home Care experience.

Responsibilities

• Deliver care in the home setting to assigned patients following nursing plans and protocols.
• Teach patients and caregivers under supervision of a RN.
• Collaborate with RN on patient status and treatment responses.
• Administer medications and evaluate effectiveness as applicable.

Responsibilities

The Home Health Field LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :

• Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse
• Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse
• Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status
• Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Home Health Field LPN - Requirements

• BLS (Basic Life Support)
• LPN (Registered Nurse) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
• Required Associate's or Diploma Nursing
• Required 1 year Licensed Practical Nurse
• Required 1 year Home Care
