Job summary

BayCare HomeCare invites a skilled Home Health Field Physical Therapist to join our team in Largo, FL. This full-time role focuses on patient-centered home visits, delivering quality physical therapy in patients' homes with a Monday–Friday schedule (8:00 a.m.

– 5:00 p.m.) and occasional weekends. You will evaluate patients, coordinate with a multidisciplinary team, and develop discharge plans to promote independence and optimal recovery in the home setting.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Florida Physical Therapy license.
• Minimum 1 year of Physical Therapist experience.
• BLS certification.
• Florida driver's license within 30 days of hire.
• Master's Degree in Physical Therapy preferred.

Responsibilities

• Evaluates and develops treatment programs for home health patients.
• Provides clinical direction for Physical Therapy Assistants.
• Plans and manages home health services to meet patient needs and quality standards.
• Manages activities of the multi-disciplinary care team.
• Monitors therapeutic services and communicates with all disciplines.
• Develops and carries out discharge plans with patient and family.
• Interprets agency policy to patients and families.
• Reviews patient visits weekly and informs supervisor and scheduler.
• Informs scheduler of schedule changes and communicates caseload and issues to supervisor.

• Home visits in assigned service area: Largo, St. Petersburg, Pinellas Park - surrounding areas assigned by zip code.

Responsibilities

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.
• Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patients visits weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits.
• Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

HomeCare Physical Therapist Requirements

• Bachelor's Degree Physical Therapy
• Physical Therapist State of Florida License
• 1 year Physical Therapist experience
• BLS (Basic Life Support)
• Driver's License (Florida driver's license is required within 30 days of hire)
• Preferred Master's Degree Physical Therapy
