Job summary

BayCare HomeCare is seeking a dedicated Home Health Field LPN to provide in-home patient care in the New Port Richey area. The role offers a full-time schedule (8:00 a.m.–5:00 p.m.) with occasional weekends, focusing on delivering nursing care in patients' homes and ensuring adherence to care plans with RN supervision.

Ideal candidates have an active LPN license, BLS, and at least one year of home care experience, plus a valid Florida driver’s license.

Qualifications

• LPN license required; BLS certified; Florida license expected within 30 days of hire.
• At least 1 year of licensed practical nurse experience, preferably in home health.
• Valid Florida driver’s license is required.
• Associate’s degree or nursing diploma is required.
• Excellent patient care, documentation, and communication skills.

Responsibilities

• Deliver patient care to assigned home patients, including IV infusions, infusion pumps, and tube feedings under RN supervision.
• Identify, implement, and maintain teaching plans for patient and caregivers under RN direction.
• Follow the plan of care and collaborate with RN regarding patient response and status changes.
• Utilize nursing knowledge in medication administration and evaluation of effectiveness.

Responsibilities

The Home Health Field LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :

• Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse
• Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse
• Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status
• Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Home Health Field LPN - Requirements

• BLS (Basic Life Support)
• LPN (Registered Nurse) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
• Required Associate's or Diploma Nursing
• Required 1 year Licensed Practical Nurse
• Required 1 year Home Care
