Job summary

BayCare HomeCare is seeking a dedicated Home Health Field LPN to deliver high-quality care in patients' homes within the Tampa Bay area. This full-time, exempt role features an 8:00 AM–5:00 PM schedule, with on-call duties and occasional weekends, supporting patient independence and recovery.

You will administer medications and IV therapies under RN supervision, develop teaching plans for patients and caregivers, and collaborate with the nursing team to ensure optimal outcomes.

Qualifications

• BLS (Basic Life Support) certification required.
• LPN license for Florida in good standing.
• Experience delivering home-based nursing care preferred.

Responsibilities

• Provide hands-on nursing care in patients' homes per plan of care.
• Administer medications, IV infusions, and infusion pumps as supervised.
• Educate patients and caregivers; adjust care plans with RN guidance.
• Collaborate with RN on patient status and responses to treatment.

The Home Health Field, LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :

• Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse
• Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse
• Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status
• Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Required Certifications and Licensures:

• BLS (Basic Life Support)
• LPN (Licensed Practical Nurse) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

Education:

• Required Associate's or Diploma Nursing

Experience:

• Required 1 year Licensed Practical Nurse
• Preferred 1 year Home Care
