Job summary

Prime Healthcare's Holy Family Medical Center in Chicago, IL, seeks a Field Liaison to coordinate post-hospitalization services. You will assess physical, psychological, and social needs, promote survivorship, and conduct community education and outreach within the Des Plaines area.

Qualifications

• Current and valid state LVN, PT, OT, RRT or RN license preferred.
• Current BLS certificate upon hire and maintain current.
• Minimum of five (5) years of healthcare experience preferred.
• Bachelor’s degree in healthcare or closely related field preferred.

Responsibilities

• The Field Liaison will coordinate services and identify patients through the health system that may qualify for post hospitalization services.
• Assist with identifying patients and assist the hospital with referrals and prior authorization for services aligned with facility standards.
• Develop relationships and serve as an educational resource for referring providers, patients and family members.
• Coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care.
• Assesses the physical, psychological, and social needs of the patient enhancing patient outcomes, increasing satisfaction, and reducing cost of care.
• Responsible for community education and screening as well as monitoring community outreach and promoting survivorship.

Education

State LVN, PT, OT, RRT or RN license

Bachelor’s degree in healthcare or related field

BLS certificate

Minimum five years healthcare experience
