Job summary

Navian Hawaii is seeking a compassionate Hospice Aide to support patients and families in their homes and community settings across Oahu. You will assist with personal care, daily living activities, mobility, and comfort, while documenting care and communicating with the care team. Travel to patient locations is required.

The role emphasizes dignity, empathy, and teamwork within an interdisciplinary team, with a schedule mainly Monday to Friday and potential flexibility for weekends or evenings.

Qualifications

• Valid Certified Nurse Aide in the State of Hawai’i.
• 1 year of direct patient care experience required.

Responsibilities

• Assist patients with personal hygiene and activities of daily living (bathing, mouth care, skin care, dressing, feeding).
• Assist with ambulation, transfers, ROM exercises, and positioning for comfort.
• Document care per agency protocol and report changes to the Nurse Case Manager.
• Provide a safe, sanitary environment and help with equipment, nutrition, and safety.
• Collaborate with the Interdisciplinary Team and participate in conferences.
• Travel to patient homes, facilities, and offices throughout Oahu as required.
