Choice Health At Home is seeking a Home Health Field RN in the Ocala area. The role focuses on comprehensive in-home nursing care through assessments, planning, implementation, and evaluation, with collaboration across care teams to support recovery and chronic disease management.

The RN will assess patient needs, determine care scope, educate patients/families, and ensure continuity of care in home settings while adhering to infection control standards.

Competenze

• Current RN license in good standing.
• Valid Driver's License required.
• CPR Certification preferred.

Mansioni

• Conduct initial and ongoing assessments of patients in home health settings.
• Develop, implement, and document patient-specific care plans.
• Educate patients and families on self-care, nutrition, and disease management.
• Coordinate with physicians and care teams to optimize outcomes.
• Provide clinical direction to Home Health Aide and LPN/LVN.
• Maintain accurate progress notes and communicate changes to the care team.
