Choice Health at Home is seeking a Home Health Field RN to deliver patient-centered care in patients' homes in Clearwater, FL. This full-time role requires a Florida RN license, CPR, and a valid driver's license.

You will assess, plan, implement, and evaluate care with a collaborative team to promote recovery and independence. The position follows a pay-per-visit model, requiring strong clinical judgment, education for patients and families, and coordination with Home Health Aide and LPN/LVN

Qualifikationen

• Current RN license required.
• CPR certification preferred.
• Valid driver's license required.

Aufgaben

• Perform initial and ongoing comprehensive home health assessments and treatments.
• Develop, implement, and document the plan of care for each patient.
• Educate patients and families on self-care, nutrition, and home health needs.
• CoordinateCare with interdisciplinary team and third-party payors.
• Provide directions to Home Health Aide and LPN/LVN as needed.
• Document progress and communicate changes to physicians.
• Monitor cases for compliance with requirements and inform team of patient changes.
