Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable, including home, nursing facilities, and 'pop-up' clinics.

The Nurse Practitioner will primarily work in non-clinical settings, providing medical care to diverse patient populations, including specific focus areas such as adult, geriatric, pediatric, or women’s health.

Responsibilities

• Perform comprehensive medical assessments, order tests, formulate treatment plans, and coordinate with specialists.
• Provide general medical care and care coordination for various patient groups.
• Conduct evaluations, address primary care complaints, and establish diagnoses.
• Order diagnostic tests judiciously, knowing when physician oversight is necessary.
• Create and implement medical care plans, schedule visits, and coordinate post-discharge care.
• Perform face-to-face visits in various settings, including via alternate modalities as needed.
• Drive up to 120 miles daily within the assigned region, with potential overnight stays for extended projects.
• Obtain and maintain cross-state licensing as required.
• Prescribe medications, perform procedures, and document care timely in electronic records.

Qualifications

Required Education: Master's degree in family health from an accredited nursing program.

Required Experience/Skills: Proficiency with Word, Excel, and Electronic Medical Records.

Required Licenses/Certifications: Active national certification (e.g., AANP, ANCC), current state license as a Family Nurse Practitioner, BLS certification, and a valid driver’s license.

Preferred Qualifications

• 3-5 years of experience as an RN or Nurse Practitioner, especially in home or community health.
