The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by the physician and in compliance with laws, regulations, and agency policies.

Responsibilities include:

• Coordinating the total plan of care with healthcare professionals and ensuring continuity of care through effective communication with physicians, patients, families, and community resources.
• Conducting initial and comprehensive nursing evaluations, developing care plans within state guidelines, and submitting accurate documentation in collaboration with physicians.
• Performing patient assessments and developing appropriate care plans at designated times.
• Ensuring patients meet home health eligibility and medical necessity guidelines as defined by payer sources.

License and Certification Requirements:

• Current RN licensure in Idaho
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to a reliable vehicle or public transportation
