Job Description
The Nurse Practitioner will work primarily in non-clinical settings, providing medical care to diverse patient populations. Responsibilities include performing comprehensive assessments, ordering tests, formulating treatment plans, consulting specialists, and documenting care. Some programs may focus on specific populations such as adults, geriatrics, pediatrics, or women’s health.

Job Duties

  • Provide general medical care and care coordination for various patient groups.
  • Perform evaluations, including history and physical exams, for care gaps and preventive assessments.
  • Address chronic and acute complaints, assessing urgency.
  • Establish and document diagnoses.
  • Seek specialty consultations when needed.
  • Order and interpret diagnostic tests judiciously, working within resource limitations.
  • Recognize when patient needs exceed scope and seek physician oversight.
  • Create and implement medical care plans.
  • Schedule and conduct patient visits in various settings, including home and facilities.
  • Participate in community clinics and outreach programs.
  • Drive within assigned regions, including extended mileage and overnight stays as needed.
  • Maintain licensure in multiple states if required.
  • Collaborate with colleagues, participate in meetings, prescribe medications, and document in electronic medical records.
  • Assist with urgent lab and pharmacy inquiries, and perform procedures as appropriate.
  • Perform physical tasks, including walking stairs and carrying equipment.

Job Qualifications

Required Education: Master's degree in family health from an accredited nursing program.

Required Experience/Skills: Proficiency with Word, Excel, and EMR systems.

Licenses and Certifications:

  • Active national certification from the American Academy of Nurse Practitioners or ANCC.
  • Current state license as a Family Nurse Practitioner.
  • Basic Life Support certification.
  • Valid driver’s license.

Preferred Experience:

  • 3-5 years as a Registered Nurse/Nurse Practitioner, especially in home or community health.
  • Experience managing chronic conditions in underserved populations.
  • Multilingual abilities are a plus.
  • Skills in immunization and point-of-care testing.

Pay Range: $80,412 - $188,164 annually. Actual compensation varies based on location, experience, education, and skills.
