Responsibilities

The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

Responsibilities include coordinating the total care plan with other healthcare professionals, ensuring continuity of care by communicating with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.

Conduct initial and/or comprehensive nursing evaluations, determine the primary focus of care, develop the care plan following state-specific guidelines, and ensure accurate, complete, and timely documentation in collaboration with physicians.

Perform comprehensive assessments and develop appropriate care plans at designated times.

Verify patients meet home health eligibility and medical necessity criteria as defined by the payer source.

Education and Experience

License Requirements:

  • Current RN licensure in Idaho
  • Current CPR certification
  • Valid driver’s license, vehicle insurance, and access to reliable transportation (vehicle or public transit)
