Job summary

San Diego Center for Children seeks a Lead Care Manager to coordinate Enhanced Care Management, provide direct services, and navigate health and social services for clients in the Mission Valley area. You will work within a multidisciplinary team to promote wellness, independence, and access to care.

The role emphasizes offsite and in-home outreach, collaboration with families, and translating documents as needed to support Spanish-speaking clients.

Qualifications

• Experience using EMR/EHR systems and care management tools.
• Ability to conduct home visits and outreach to clients.
• Strong verbal and written communication with diverse populations.
• Excellent organizational skills and the ability to manage a personal schedule.

Responsibilities

• Assess client health and social needs across physical, mental health, and social domains.
• Develop and implement Individual Care/Health Action Plans.
• Provide services in clients' homes, clinics, telehealth, or community settings.
• Coordinate with health care professionals and community resources.
• Document activities and update goals regularly.
