Job summary

Victor in Riverside, CA is seeking a Lead Care Coordinator (LCC) to provide direct service and coordinate care management within the Enhanced Care Management (ECM) program. The LCC leads care teams, develops care plans, and helps clients access timely, cost-effective services.

This role requires 3+ years in health/social services, bilingual English/Spanish, strong communication, and ability to work in office, telehealth, or field settings.

Qualifications

• Must be bilingual in English/Spanish.
• Excellent customer relations and written communication.
• Proficient with Microsoft Word and Excel.

Responsibilities

• Assesses client needs across physical health, mental health, SUD, housing, vocational/employment, wellness, and supports.
• Oversees the development and implementation of the Individual Care Plan/Health Action Plan.
• Provides services where the client lives, seeks care, or is most accessible (office, telehealth, or field).
• Connects clients to community-based services and supports.
• Coordinates and advocates with health care professionals with consent and authorization.
