Job summary

Maximus is seeking a Field Clinical Assessment Coordinator to join the Outreach & Assessment team. This hybrid role involves conducting person-centered outreach interviews and clinical evaluations to determine needs, risks, and services, with travel across Cook County and the Chicago metro area.

Ideal candidates have 5+ years in medical or behavioral health, a relevant master's degree or active license, and strong interviewing and documentation skills.

Qualifications

• High school diploma or GED required; 2+ years of relevant professional experience.
• Master’s degree in counseling, social work, psychology, or related field OR active, unrestricted license.
• Minimum 5 years’ experience in medical or mental health.
• Willingness and ability to travel up to 85% throughout the greater Chicago metropolitan area, with occasional travel outside the region based on business needs.
• Strong interviewing, documentation, and interpersonal skills.

Responsibilities

• Complete medical and/or behavioral health assessments using standardized tools.
• Conduct on-site, video, and/or telephonic assessments as required.
• Document assessment findings accurately in program systems.
• Travel to client sites; support program delivery with in-person assessments.
• Educate individuals about rights and next steps; explain the Outreach & Assessment process.
