Custom Case Management; A VONA Company is seeking a Field Case Manager (RN) to coordinate rehabilitation needs for injured workers within workers' compensation. You will assess medical conditions, plan treatment with providers, and advocate for timely, cost-efficient return-to-work.

The role requires independent clinical judgment, HIPAA compliance, and collaboration across departments and with employers. Multilingual skills are a plus; ongoing licensing and professional development are expected.

Qualifications

• RN license required.
• Experience in case management is required, workers' compensation or occupational health preferred.
• Strong knowledge of medical terminology and clinical practices.
• Ability to exercise independent clinical judgment and make decisions impacting case outcomes.
• Experience in hospital or rehabilitation settings.
• Familiarity with HIPAA regulations and confidentiality standards.

Responsibilities

• Independently assess the injured worker's medical condition and develop a coordinated treatment plan with healthcare providers.
• Cultivate and maintain strategic relationships to generate ongoing referral opportunities.
• Collaborate on individualized care plans across departments and external providers.
• Coordinate services across departments with cost-efficiency and regulatory compliance.
• Analyze client needs and recommend interventions affecting outcomes and resource allocation.
• Exercise independent clinical judgment when prioritizing client needs and adjusting plans.
• Coordinate with leadership on policy and procedural updates; engage with executive-level staff on complex cases.
• Maintain timely, professional communication with injured workers, medical professionals, employers, and insurers.
• Prepare monthly progress reports and detailed documentation per company standards.
