• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Contribute to quality of care by consulting with clinicians and conducting case conferences.
• Identify and escalate quality issues through established channels.
• Support member engagement and empower independent medical/healthy lifestyle decisions.
