Job summary

Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will travel locally to client homes in Virginia, guiding families through Medicaid Waiver programs and coordinating care plans. A strong communicator with a heart for service is essential.

No degree is required, and experience with disabilities or the elderly is valued. You will work remotely from a laptop/tablet and help families live safe, healthy lives.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; emphasis on heart and experience.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• Must have a valid driver’s license and reliable vehicle.
• Great written and spoken communication.
• Able to pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients at their homes.
• Be the go-to guide for families navigating waiver programs.
• Write plans of care and assessments that actually help families.
• Troubleshoot challenges like service authorizations and timesheets.
• Build lasting relationships with families based on respect and trust.
• Work remotely from your laptop/tablet.
