Humana is seeking a Case Manager to support Wisconsin Family Care members in the Hayward/Park Falls area. You will coordinate with a Field Care Nurse to develop and implement a comprehensive Member Care Plan (MCP) and conduct regular in-person and phone check-ins.

The role requires a Bachelor’s degree in human services or related field, with 1 year of relevant experience or 3 years if degree is in another field, plus a valid driver's license and insurance.

Competenze

• Bachelor’s degree in human services or related field required; 1 year experience with frail elders or adults with disabilities, or 3 years in lieu
• Valid state driver’s license and proof of personal vehicle liability insurance (100/300/100) required

Mansioni

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP)
• Coordinate with a Field Care Nurse to address health and safety needs in least restrictive environment
• Conduct face-to-face social assessments at enrollment and at least every six months
• Conduct quarterly in-person visits and monthly phone contact with members
• Arrange non-medical support services (social integration, housing, employment, etc.)
• Maintain accurate documentation including case notes and MCP updates
