Job summary

CVS Health is seeking a Case Management Analyst for a field-based role in the Virginia area (Arlington, Alexandria, Annandale, Springfield, Falls Church) to assess, plan, implement, and coordinate case management for members. You will work face-to-face and by phone to evaluate medical needs and develop proactive action plans, following network management and clinical guidelines.

This full-time role offers 40 hours weekly, up to 75% travel, competitive benefits, and eligibility for CVS Health

Qualifications

• 2+ years' experience in behavioral health, social services or related field.
• Bachelor’s Degree or non-licensed master level clinician in behavioral health or human services preferred (psychology, social work, counseling, etc.).

Responsibilities

• Assess, plan, implement, and coordinate case management activities with members.
• Evaluate medical needs and formulate proactive actions to improve short- and long-term wellness.
• Follow CVS Health policies and clinical coverage guidelines.
