Qualifications

• Must reside in Annandale, Falls Church, Springfield, Arlington, Alexandria VA area
• Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy
• 2 or more years' experience in behavioral health, social services or appropriate related field

Responsibilities

• Assess, plan, implement and coordinate all case management activities with members to evaluate medical needs
• Develop proactive course of action to address issues and enhance short and long-term outcomes
• Align services with network management and clinical coverage policies
