• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, families, and clinicians.
• Monitor care plan effectiveness and adjust as needed.
• Promote integrated services across LTSS and behavioral health.
• Assess medical necessity and authorize waiver services.
