Job summary

Molina Healthcare is seeking a Care Manager to support LTSS-specific activities, collaborating with a multidisciplinary team to coordinate integrated member care across the continuum. You'll help ensure progress toward outcomes and contribute to cost-effective care delivery.

Responsibilities include comprehensive assessments, waiver enrollment, care plan development, and ongoing monitoring. Travel may be required to service members in various settings across Illinois.

Qualifications

• 2+ years health care experience, including LTSS, and 1 year in care management or related settings.
• Clinical licensure/certification required only if state contract requires it.
• Bachelor’s degree may be required in some states depending on contract.
• Valid driver’s license and reliable transportation for job travel.
• Experience with diverse populations and multiple settings is preferred.
• Proficiency with Microsoft Office and navigating online portals/databases.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits when required.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers, and clinicians.
• Monitor care plans and adjust interventions to achieve goals.
• Promote integration of services across LTSS and behavioral health for continuity of care.
• Assess medical necessity and authorize waiver services.
• Collaborate with ICT for approvals/denials and care coordination.
• Educate and motivate members using motivational interviewing and clinical guidelines.
• Identify barriers to care and coordinate psycho/social/financial support.
• Identify incidents and develop prevention plans to safeguard member welfare.
• Travel up to 25–40% as required by state contracts.
