Job summary

Molina Healthcare in Illinois is seeking a Care Coordinator for LTSS to support care management and coordinate services across the continuum for high-need members. You will collaborate with an interdisciplinary team to develop and monitor comprehensive care plans, ensuring progress toward outcomes and appropriate use of waiver services.

Responsibilities include conducting member assessments, facilitating waiver enrollment, and promoting integration of behavioral health and LTSS with community

Qualifications

• At least 2 years health care experience including LTSS/care management.
• Licensure/certification required if state contract/regulation mandates.
• Bachelor's degree may be required in some states.
• Valid driver's license and reliable transportation for travel.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitate comprehensive waiver enrollment and disenrollment processes.
• Develop and implement care plans with waiver service plan in collaboration with members, caregivers, physicians, and others.
• Monitor care plan effectiveness and document interventions and goals.
