Qualifications

• At least 2 years of health care experience including LTSS exposure.
• Clinical licensure/certification required only if mandated by state contract.
• Valid driver's license and reliable transportation for travel.
• Ability to work independently and with diverse populations.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits when required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with care team and members to address needs and goals.
• Monitors care plan effectiveness, documents interventions and adjusts as needed.
• Promotes integration of LTSS, behavioral health, and community resources to ensure continuity of care.
• Assesses medical necessity and authorizes waiver services; coordinates ICT meetings.
• Identifies barriers to care and provides support for psycho/social, financial, and medical obstacles.
• 25-40% estimated local travel may be required based on state/contractual requirements.
