Qualifications

• At least 2 years of health care experience, including LTSS, and 1 year in care management.
• Licensure and/or certification required only if state contract mandates it.
• Bachelor's degree may be required depending on state/contractual requirements.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources and ability to work with diverse populations.
• Strong communication, time-management and self-motivation.
• Experience with substance use disorders may be required in some states.

Responsabilités

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and support networks.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of LTSS with behavioral health and community resources.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources.
• Facilitates ICT meetings for service approvals.
• Educates and motivates members using motivational interviewing.
• Identifies barriers and coordinates psychosocial, financial, and medical support.
• Identifies critical incidents and develops prevention plans.
• Collaborates with licensed care managers as needed.
• 25-40% local travel may be required.
