Job summary

FreedomCare in Indiana is seeking a Care Manager to join our field-based team. You will conduct patient orientations, assess home care needs, and train caregivers to ensure compassionate, high-quality care for loved ones at home.

Travel throughout the Bloomington area is required, with mileage and expenses reimbursed. The role emphasizes collaboration with care teams, case note entry, and ongoing process improvements to support patients and families. Base pay is $25–$28 per hour, plus benefits.

Qualifications

• 2+ years of experience working with frail or elderly adults and caregivers.
• Experience in medical or case management environment is preferred.
• Valid driver’s license with an insured vehicle for travel within Indiana.
• Excellent verbal and written communication, organizational and time management skills.

Responsibilities

• Evaluate the home care needs of the client.
• Assess the environment for safety and suitability.
• Assess caregiver fitness to provide care.
• Conduct home orientations and sign paperwork.
• Perform initial assessments during orientations.
• Conduct periodic supervisory visits for on-care cases.
• Train caregivers and patients as required per case.
• Engage in interdisciplinary meetings with care managers, caregivers and patients.
• Communicate impressions of cases to appropriate staff.
• Critically assess procedures and implement improvements for evaluations and training.
• Travel within the state of Indiana with mileage/expense reimbursement.
