Job summary

FreedomCare is seeking a Care Manager in Georgia to support home-based patients across the Atlanta Metro Area. You will conduct orientations, perform patient assessments, and train caregivers to ensure high-quality in-home care.

The ideal candidate has 2+ years of experience with frail or elderly adults, strong communication, and organizational skills. This role offers flexible days and travel across Georgia with a pay rate of $50 per completed field visit.

Qualifications

• 2+ years with frail or elderly disabled adults and caregivers.
• Field experience in medical or case management encouraged.
• Experience with data input and case notes; Salesforce a plus.
• Home care experience preferred.
• Knowledge of ADL/IADL assessments.
• Valid driver’s license with an insured vehicle for travel within GA.
• Strong communication and organizational skills.
• Proactive, problem-solving mindset.
• Proficient in Microsoft Word, Excel, Outlook.

Responsibilities

• Home orientations involving signing paperwork.
• Initial assessments during orientations.
• Periodic supervisory visits/check-ins for on-care cases.
• Training caregivers/patients as required.
• Interacting with caregiver and stakeholders.
• Interdisciplinary meetings with care managers, caregivers, and patients.
• Communicate impressions to appropriate staff.
• Assess procedures and implement improvements; train caregivers.
• Wear multiple hats as operations develop.
• Travel within the state of Georgia (Atlanta Metro Area)
