Job summary

Western-Slope-in-Home-Car is seeking a Field Supervisory Assistant to visit clients and caregivers in the field, ensuring care plans meet client needs and coordinating follow-up as needed. You will manage intake calls, route or solve issues, and support scheduling tasks within approved scope.

This role emphasizes communication, organization, and timely reporting to care coordinators. Ideal candidates have a high school diploma or GED with home care experience, strong English/Spanish

Qualifications

• High School Graduate with two years of Home Care experience.
• Excellent verbal and written communication; bi-lingual in Spanish/English a plus.
• Must pass criminal background check.
• Ability to work independently with sound judgment.
• Basic computer skills and the ability to learn inERSP.
• Typing speed 60 WPM or greater; 70–90 WPM a plus.
• Must have a car and be available for monthly on-call duties outside office hours.

Responsibilities

• Visit clients, family, and caregiver staff to ensure care plans meet client needs.
• Document changes and report to care coordinators on status or service changes.
• Participate in 90-day client care plan evaluations as needed.
• Handle first/second incoming calls for QA and 24/7 call center support.
• Route calls or solve problems; manage shifts and call-outs per schedule.
• Assist with front-line communications and delegate tasks as directed.
