Job summary

Qualifications

• BA/BS degree required.
• Minimum of 1 year of experience working directly with people related to the program population or equivalent background.

Responsibilities

• Perform telephonic or face-to-face history and program needs assessments.
• Identify members with potential clinical health care needs and coordinate with the clinical healthcare management and interdisciplinary team.
• Manage non-clinical needs of members to ensure cost-effective utilization of services.
• Establish short and long-term goals in collaboration with the member, caregivers, family, and physicians.
• Identify members who would benefit from expanded services.
• Travel to worksites and designated locations as necessary.
