Qualifications

• BA/BS degree required or equivalent.
• 1 year of experience working with the program population or related community-based organizations.
• QMHP, MSW or BSW degree preferred.

Responsibilities

• Manage member's individual service plan within the scope of position.
• Perform telephonic or face-to-face history and needs assessments to identify and coordinate program needs.
• Coordinate cases with the clinical healthcare management and interdisciplinary team to provide care coordination.
