Job summary

Caringhand Home Health Solutions, Inc. is seeking a compassionate RN for field-based, per diem home visits in California. The role involves assessing patients at home, developing care plans, administering medications, and coordinating with physicians and families.

Travel to patients' homes is required, and on-call duties may apply. The RN will supervise home health aides, conduct discharge planning, and ensure high-quality patient care in a home setting.

Qualifications

• Graduate of an accredited school of nursing.
• Current, active Registered Nurse (RN) license in California.
• At least 1–2 years of recent acute care experience in an institutional setting.
• Current CPR certification.
• Valid driver's license and access to a reliable vehicle insured in accordance with state and organizational requirements.
• Excellent observation and clinical assessment skills.
• Strong verbal and written communication skills.
• Ability to work independently and coordinate patient care responsibilities.

Responsibilities

• Conduct initial patient and family assessments to determine home health care needs.
• Regularly reassess patients’ nursing needs and evaluate changes in their conditions.
• Initiate and revise individualized plans of care as patient status and needs change.
• Administer medications and treatments as prescribed by the physician.
• Counsel patients and families regarding nursing and related healthcare needs.
• Provide patient and caregiver education based on assessments and the established plan of care.
• Identify discharge planning needs during care planning and implement appropriate arrangements before patient discharge.
• Serve as case manager when assigned by the Clinical Supervisor or Nursing Supervisor.
• Prepare accurate clinical notes and update the primary physician as needed and at least every 60 days.
• Coordinate with community healthcare professionals and other relevant parties to support the patient’s plan of care.
• Participate in on‑call duties according to organizational policy.
• Instruct, supervise, and evaluate home health aide services provided to patients every two weeks.
