Caringhand Home Health Solutions, Inc. seeks a compassionate Registered Nurse (RN) for field-based, per diem home health visits in California. You will assess patients in their homes, develop care plans, administer medications, educate families, and coordinate with physicians to ensure quality care.

The role requires active CA RN licensure, CPR, a valid driver's license, and 1–2 years of acute care experience. Travel to patient homes is expected, with flexible scheduling.

Qualifications

• Graduate of an accredited school of nursing.
• Current, active Registered Nurse (RN) license in California.
• 1–2 years of recent acute care experience in an institutional setting.
• Current CPR certification.
• Valid driver's license and access to a reliable vehicle insured as required.

Responsabilités

• Conduct initial patient and family assessments to determine home health care needs.
• Regularly reassess patients’ nursing needs and changes in condition.
• Develop and revise individualized plans of care.
• Administer medications and treatments as prescribed.
• Coordinate care with physicians and other healthcare professionals.
• Supervise and evaluate home health aides as needed.
• Prepare clinical notes and update physicians at least every 60 days.
